Life as an attending can be ok but I really feel like it's worth pursuing a fellowship for multiple reasons: You won't get dumped on. You can (potentially) make more money. You are less replaceable. A lot of hospitalists could be replaced with mid levels that pan-consult.

Rheumatology median salary coming out of fellowship right now is $240k. That’s including academic positions that pay less. Most advertised jobs are 4-4.5 days of work, around 8-4 or roughly that same amount of hours. This post makes it seem like the average rheum doc is making $180k a year on an average of 40 hrs. Hospitalist jobs are everywhere and they pay pretty well. The going rate is around 250k for ~20-22 weeks. EM takes a different mindset to do.
